LAPIS UFO Sighting Report Form (UK ONLY)

Firstly, thank you for reporting your ufo sighting to LAPIS. We would appreciate it if you could take a few minutes of your time to complete the following ufo report form, which will be used as part of our ongoing investigations into ufo sightings in North West England and especially the Blackpool & Preston areas of Lancashire.

We don't guarantee results but we will endeavour to investigate your ufo sighting to the best of our ability and attempt to identify the object you witnessed. Any information given by you in this report, or any subsequent follow-up investigation will be treated in the strictest confidence - guaranteed!

Please complete as many boxes as possible in the ufo report form below - you DON'T have to complete them ALL but the more information you give the more it will help our investigations.

WITNESS STATEMENT
Please give a description of the sighting as you remember it. Include brief details of what you were doing at the time of the sighting (e.g. driving a car, mowing the lawn, walking along the street). Please be as descriptive as possible.

Describe the colour or colours of the object:

How large do you think the object appeared to you compared with the following at arms length:

How large did the object appear compared with a bright star / the moon?

Did the object appear solid?

Would you say that the edges of the object were sharp or fuzzy?

Did the object make any sound?

What background noise could be heard?

How did you become aware of the object (e.g. suddenly appeared, caught sight of something out of the corner of your eye)?

In which compass direction was the object first observed?

Please indicate the approximate angle of elevation of the object (e.g. 45 degrees above the horizon).

For how long in total was the object visible?

Did the object at any time: hover / change shape / pulse / brighten-dim / rotate / flicker / change colour / explode / emit smoke or do anything that seemed unusual?

During the ufo sighting, what movements, if any, did the object make (e.g. darting around, falling leaf, zigzagging)?

Did the object move in front of or behind anything?

Did you smell anything unusual at the time of the sighting?

How did the object disappear (e.g. appeared to vanish instantly, shot off at great speed, seemed to evaporate or fade)?

In which compass direction was the object last observed?

Did you notice any unusual effects before, during or after the ufo sighting (e.g. sickness, dizziness, redness of skin, disorientation)?

Did you get the feeling that the object was aware of you?

At any point during the ufo sighting did you have a feeling of deja vu (an uncanny feeling that you had seen this object before or that something similar to this incident had happened to you before)?

Immediately after the ufo sighting, was there any lost or missing time you could not account for (e.g your journey seemed to take twice as long)?

Was there anything unusual about the ufo sighting which has not been covered in any of the questions above? Mention anything you feel could be attributed to the sighting no matter how trivial or silly you may think it sounds - it could be important.

Do you know if anyone else saw this object (at the same time / location)?
